Where To Begin?

Once an applicant is assessed by The Department of Social Development they will be placed on the Nursing Home List. This list is distributed to the nursing homes in your region. You and/or your family are expected to choose three nursing homes of your preference. For more information about this process please click the link below.

Application Process

If you are thinking about our Home for your loved one we encourage you to come visit. We are happy to take families and potential residents on a tour of our Home, you may schedule a tour by calling (506) 643-6003. We have applications available at the front desk or you can print one below.

Application

Completed applications can be returned to the home in person Monday -Thursday between 9:00am-4:00pm, by mail or email to resident@churchhomenet.com.
